Section 33

Whenever for any reason, including the completion of his contract of apprenticeship, an apprentice ceases to be employed by the employer, the employer shall supply the apprentice with a statement, in such form as the Commissioner may determine, setting forth the service of the apprentice.

The employer shall forward a copy of such statement to the

Commissioner, who shall endorse a note of the service on the contract of apprenticeship.
