Section 2
(1)
An application under subsection (1) of section 7 of the Order for the grant of a licence shall be in Form 1 of the First Schedule and shall also contain such other information as the Commissioner may require.
(2)
An application under section 12 of the Order for renewal of a licence shall be in Form 2 of the First Schedule and shall also contain such other informa-tion as the Commissioner may require.
