Section 40
(1)
Whenever for any reason including the completion of his contract of apprenticeship an apprentice ceases to be employed by an employer it shall be the duty of the employer to supply the apprentice with a statement in the prescribed form setting forth the service of the apprentice.
(2)
The employer shall forward a copy of such statement to the
Commissioner, who shall endorse a note thereof on every copy of the contract of apprenticeship submitted to him for that purpose by any of the parties to such contract.
